Observation: GE reach in cooler in the back prep room is not maintaining proper cold holding temperature. (50 ambient). Employee moved all foods out of the cooler that were placed in this morning to the walk in cooler. Owner will monitor the temperature. Cooler was holding temperature days prior to visit.

Observation: Employee drinks with no lids stored in kitchen. Employees placed lids on drinks.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: Thermometer not available in the GE cooler.

Observation: Unused table mounted can opener has debris build up on the blade and needs to be cleaned or removed.

Observation: Refried beans(48F), Sour cream cups(52F), Soup in bags(46F), Sausage(47F) stored in the GE cooler was not maintaining cold holding temperature. Employees stated the product was in the cooler since yesterday. All product was discarded.

Observation: Observed employee handle ready to eat food items with bare hands. Discussed bare hand and glove requirements with staff. Staff put on new gloves.

Observation: No quat test strips available to test sanitizer concentration.

Observation: No paper towels available at the kitchen hand sink. Correct by employee.

Observation: Spray bottle filled with orange degreaser was labeled as a sanitizer solution. Employee relabeled the bottle correctly.

Observation: Employee preparing foods was wearing no kind of hair restraint.

Observation: Inside of the main kitchen prep cooler needs to be cleaned out. Inside of the GE cooler in the back prep room needs to be cleaned out.

Observation: Heavy dust debris build up on kitchen ceiling around vents. Grease and debris build up on the floor under the kitchen fryer.

Observed ice scoop stored on pop syrup box in the backroom by the ice machine.
- Bare hand contact with ready to eat foods
Observed food handlers touching lettuce and pickles with their bare hands (food product that would not receive further heat treatment). This violation was cited at the last routine inspection-Risk Control Plan must be implemented to address this repeat violation.

The floors in the walkin cooler and walkin freezer under shelving are excessively soiled with food residue and garbage (cup lids, etc.). Ceiling areas in the kitchen around ventilation vents have a build up dust/residue.

Observed food handler preparing food in the kitchen was not wearing a hair restraint to effectively restrain the hair on top of their head.
- Common name-working containers
Observed one unlabeled bottle of a chemical in the kitchen that the staff could not identify at the time of inspection. Corrected at time of inspection.

Observed unpackaged frozen pizza crusts stored in direct contact with the shelving in the upright refrigerator/freezer. Kitchen manager disposed of unprotected product. Corrected at time of inspection.
