The purpose of this visit was to conduct a routine inspection.
ORS information given the the person in charge. Please email to me a picture certified food manager's card by September 30, 2015. Additionally email me pictures of the area surrounding the dumpster cleaned of the trash, the self closing hinge installed on the bathroom door by September 30, 2015.

  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
  • Certified Food Manager/Certificate Process (repeated violation)
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448.

    Observation: The outdoor refuse container is stored on absorbent material.
    Correction: An outdoor storage surface for refuse, recyclables, and returnables shall be constructed of nonabsorbent material such as concrete or asphalt and shall be smooth, durable, and sloped to drain.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (repeated violation)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Refuse / Area / Clean (repeated violation)
    Observation: Trash and litter were observed adjacent to the refuse container outside the facility.
    Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room
    Observation: There is no covered refuse container for the disposal of feminine napkins in the unisex restroom.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
  • Toilet Rooms, Enclosed / Self-Closing Door (repeated violation)
    Observation: Toilet room door is not provided with a self-closing door.
    Correction: Provide a self-closing hinge or closing mechanism for the toilet room door.
08/26/2015Routine
A follow up visit was made today to verify compliance with violations cited in previous inspection. A hinge to ensure door is self closing needs to be installed . There is still a small amount of trash around the dumpster area that needs to be cleaned up. I have recommended that the CFM call the trash company for a larger dumpster or to increase the frequency of pick up. Faciliy has engaged a professional pest control service and placed fly traps near the 3 VAT sink area which has reduced fly activity in the area and bait boxes near the trash area for rodent extermination. Receipts were provided that showed technician was coming by approximately every other day to check the boxes and the fly traps. Manager has been asked to continue with pest control service till vectors have been exterminated.

(see handout provided) within 15 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card will result in further enforcement action. HAVE ASKED THE CFM TO OBTAIN ORS CARD WITHIN TIME SPECIFIED AND FAX IT TO 703-653-9448.

  • Toilet Rooms, Enclosed / Self-Closing Door (repeated violation)
    Observation: Toilet room door is not provided with a self-closing door. NO HINGE PRESENT RESULTING IN THE DOOR BEING OPEN TO THE FOOD PREP AREA.
    Correction: Provide a self-closing hinge or closing mechanism for the toilet room door.ASKED MANAGER TO PROVIDE HING AND ENSURE DOOR IS SELF CLOSING.

  • Thawing / Approved Methods (corrected on site)
    Observation: Observed the following food thawing using an improper method:OBSERVED FROZEN PINEAPPLE BAGS SITTING ON A TRAY AT THE HANDSINK FOR THAWING.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process. ASKED STAFF TO MOVE THE BAGS TO THE PREP SINK UNDER RUNNING WATER.

    Observation: Observed the handsink at the BACK KITCHEN AREA BEING USED FOR THAWING PACKETS OF FROZEN PINEAPPLE.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. ASKED STAFF TO MOVE THE BAGS TO THE PREP SINK UNDER RUNNING WATER.

    Observation: Outside refuse container was uncovered. OBSERVED THAT THE DOORS OF THE TRASH DUMPSTER WERE OPEN AND TRASH AND GARBAGE BAGS WEREALSO BEING PLACED IN THE RECYCLE BIN WHICH HAD AN OPEN LID.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment. MANAGER HAS BEEN ASKED TO SECURE DOORS AND LIDS OF ALL DUMPSTERS AND PLACE ONLY RECYCLE IN THE RECYCLE BINS .

    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. ASKED MANAGER TO CLEAN AND RECAULK THE AREA.
  • Critical: Controlling Pests / Methods to Use When Pests Are Found
    Observation: Methods are not being used to control pests.OBSERVED SEVERAL DRAIN FLIES AT THE 3 VAT SINK AND EVIDENCE OF RODENTS AND RODENT BURROWS AROUND THE DUMPSTER AREAS OF THIS ESTABLISHMENT
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. CFM HAS BEEN ASKED TO ARRANGE FOR PROFESSIONAL PEST CONTROL FOR RODENTS AND FLIES AND PROVIDE DOCUMENTATION OF PEST CONTROL AT FOLLOW UP INSPECTION.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the FLOORS UNDER AND ADJACENT TO THE 3 VAT SINK AREA HAVE VISIBLE DIRT AND GRIME AND IS in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.MANAGER AND STAFF HAVE BEEN ASKED TO INCREASE THE CLEANING FREQUENCY OF THE AREA.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: OBSERVED PHILLY ROAST BEEF AT 43F, DICED CHICKEN AT 43F, SAUSAGE AT 46F IN THE DELFIELD 3 DR PIZZA PREP REFRIGERATOR.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.CORRECTED: DISCUSSED WITH CFM TO REMOVE THE ITEMS TO THE WALK IN REFRIGERATOR.
  • Equipment / Non Food / Design / Easy Clean (repeated violation)
    Observation: Soda crates found used for the following purpose: stock condiments and other items in the storage area and store squeeze bottles of dressings and sauces near the pizza oven.
    Correction: Milk crates, soda crates, and/or bread racks are not designed to store or elevate food, equipment, utensils, clean linens, and single-service items. Provide only ANSI-certified (such as NSF-listed) shelving units, storage units, or dunnage racks.
  • Plumbing / Maintained in Good Repair (repeated violation)
    Observation: Plumbing connections at the hose bib fauccet attached to the 3 VAT piping are leaking.
    Correction: A plumbing system shall be maintained in good repair.Remove the aluminium foil that is currently being used to secure the leak.
  • Handwashing Signage at Handwashing Facilities Required (corrected on site)
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.OBSERVED NO HAND WASH SIGN IN THE EMPLOYEE RESTROOM.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. CORRECTED: EHS PROVIDED FACILITY WITH A HAND WASH SIGN.
  • Physical Facilities Good Repair
    Observation: Observed that the caulking at the 3 VAT sink not maintained in good repair. OBSERVED THE CAULKING WAS IN DIREPAIR AND THERE WAS EVIDENCE OF DIRT/ MLDEW AND GRIME IN THE AREA THAT NEED TO BE CLEANED. ALSO REPLACE MISSING CEILING TILES IN THE DRY STORAGE AREA.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
