Discussed the following with the CFM: (1) The expired consume-by dated hamburger buns in the rear storage area (CFM stated they would be removed from circulation), (2) Dented can policy, (3) Defrosting the Blue Bunny chest freezer, (4) Replacing or repairing the 2-dr True RIR near the 3-compartment sink which was observed with water standing on the bottom shelf and noted towels were placed inside the unit to absorb the water (CFM stated a work order would be placed for the unit and the water & towels removed from the unit). Observation: The following equipment was observed in a state of disrepair and damaged: (1) The Reach-in Refrigerator 80171 was observed with water pooling on the bottom shelf which contained cloth towels in an attempt to contain the water. (2) The 2-door Reach-in Refrigerator 58629 was not operational (no food was in the unit).
Correction: Repair the above mentioned equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the above mentioned equipment, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: An air gap between the water supply outlet and the flood rim level mop sink was not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system. Observed the hose connected to the mop sink's faucet was below the rim of the sink. The PIC voluntarily moved the hose to provide an air gap.
Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

Observation: The doors to the employee locker room and to the utility room would not close properly and noted not maintained in good repair
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: The methods used for cooling were not adequate. Observed covered metal pans stacked on top of each other in reach-in refrigerator.
Correction: Cool foods by the following methods: 1. Placing food in shallow pans 2. Separating food into smaller thinner portions 3. Using rapid chill cooling equipment 4. Stirring the food in a container placed in an ice water bath 5. Using containers that facilitate heat transfer 6. Adding ice as an ingredient 7. Any other method that can effectively accomplish cooling.

Observation: True 2-door reach-in refrigerator was observed in a state of disrepair and damaged. The unit leaks on the inside and cloth towels were noted around the problem area on the bottom shelf of the unit.
Correction: Repair the True 2-dr RIR or restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the 2-dr RIR, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: Mops not hung up to air dry.
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

Several cold holding units including the milk boxes on the service line were elevated above the required holding temperatures however it was due to the units being opened during service which is less than 4 hours. Temperature logs indicate that the units operate properly. Defrost the ice cream units and replace frayed hot holding mitts.

Observation: The prepared ready-to-eat (RTE) Cooked fries, corn, sliced meats, and chicken breast in the refrigeration unit is not properly dated for disposition.
Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

Observation: The nonfood contact surface of the pipes and conduit lines behind equipment has accumulations of grime and debris.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Observation: Water has accumulated on the bottom shelf of the True refrigerator.
