  • Equip/Utensils/Linens/Single-Service/Invert, Cover
    Observation: Clean single serve plastic utensils were observed stored with the food-contact surface facing upward.
    Correction: Clean equipment and utensils shall be stored in mouth side down or in one direction to prevent contamination during service.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (repeated violation)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Floor , Wall & Ceiling / Design
    Observation: The carpet rug located at the restroom does not meet the standard of smooth, durable and easily cleanable
    Correction: Wall, floor, and ceiling surfaces shall be smooth, durable, easily cleanable, and non-absorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, and toilet rooms.

The purpose of today’s visit was to conduct a routine inspection. * Please remember to maintain a Certified Food Manager on site during all operating hours of the kitchen.
1. Fax copy of the CFM card (photo ID) within 10 business days.
2. Fax a copy of the invoice for the repair of the Ventilation Hood above the Vulcan Oven with 90 days.
MAINTENANCE:

*Dish Machine: N/A

  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room
    Observation: There is no cover to the feminine napkin refuse container in the ladies room stall.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Provide a covered trash can in the ladies room bathroom
  • Mechanical Ventilation Required
    Observation: Ventilation is not sufficient to keep rooms free of excessive heat. The Ventilation hood does not operate.
    Correction: Provide mechanical ventilation at a sufficient capacity to minimize soiling of walls, ceilings, and equipment

    Observation: Food stored in a location where it is subject to splash, dust or other contamination as follows: a box of chips stored under the hand sink.
    Correction: Food shall be protected from contamination by storing the food where it is not exposed to splash, dust or other contamination.

    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor. Box of chips
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Sanitizer Test Kit Required (repeated violation)
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a BLEACH,/CHLORINE test kit.
  • Sanitizing Solution Test Kit Accurate & Used (repeated violation)
    Observation: The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution.
    Correction: The operator shall be monitoring the concentration of the sanitizing solution using a test kit or other device.
  • Equipment / Non-food Contact / Visibly Clean
    Observation: The interior surfaces of the small brown refrigerator (staff unit) located in the kitchen was noted to be soiled and in need of cleaning.
    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.

    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor.---Observed following food items being stored on the floor:
    1)box of flour tortillas
    2)box of can corn
    3)box of bananas
    4)box of peaches

    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Thawing / Approved Methods
    Observation: Observed the following food thawing using an improper method:
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: pre-cook slice turkey breast 47F, milk 45F
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.---MOVE TO FREEZER TO CHILL.
