    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the hamburgers that may be served raw and/or undercooked

    Observation: Surfaces of the ice machine was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.

Any other method that can effectively accomplish cooling.

Advised follow up will be made to check consumer advisory for eggs which are cooked to order. Advised that functioning handsink must be installed in employee toilet stall or outside of it. If this is not done, toilet must be disconnected so it is not used. Follow-up to be made on or around April 9.

  • Critical: Backflow Prevention Device, When Required* (corrected on site)
    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin.
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
  • Handwashing Cleanser - Availability
    Observation: Soap was not provided at the hand washing lavatory in the kitchen handsink and in the employee toilet room.
