Teh purpose of this visit was to conduct a routine inspection. Inspection time: 11: 35 AM - 1:40 PM. Chlorine concentration final rinse dishmachine: 100 ppm. Chlorine concentration wiping cloth buckets initial/final: 0/200 ppm, 0/200 ppm.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cubed cheese/milk/salsa in 1 dr UR cooler: 51/48/47 degrees F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. NOTE: PIC voluntarily disposed of all TCS food in the cooler during the inspection.

    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Artic Air 1 dr UR cooler.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. NOTE: PIC adjusted thrermostat and the cooler had acheived a satisfacctory temperature during the inspection.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in both wiping cloth buckets with a concentration of ) ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit. NOTE: PIC had sanitizer solution of required strength added to each wiping cloth bucket during the inspection.

    Observation: Light bulb(s) in the front prep area of the kitchen are not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.

    Observation: Observed that hand soap was not provided at each handwashing sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands. NOTE: PIC had soap provided at rear handsink during the inspection, and advised handsink supply would become part of a routine checklist.

    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. NOTE: PIC had paper towels provided at both handsinks during the inspection, and advised he would make this part of a routine checklist.

    Observation: Observed that inadequate lighting was provided in the food preparation areas.
    Correction: Increase the lighting to provide a minimum of 50 foot candles in all food preparation areas. NOTE: PIC had malfunctioning light bulbs replaced druing the inspection.

  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: lettuce and onions.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods. NOTE: Discussed with PIC who had employee wash hands and don gloves. Lettuce and onions were disposed.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site) (repeated violation)
    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw steak stored in 2 dr UR cooler immediately next to baked potatoes.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. NOTE: PIC hand the storage orger corrected.

    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees.     Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food items that may be served raw and/or undercooked: (1) SAN ANTONIO BURGER (2) STEAK & MUSHROOMS IF UNDERCOOKED MEAT IS AN OPTION- OTHERWISE, CONSIDER REVISING "BROILED TO YOUR TASTE".
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. PLEASE EMAIL A COPY OF THE REVISED MENU TO THE HEALTH DEPARTMENT WITHIN 10 DAYS.

If pests are found on the premises methods shall be used to control their presence. PLEASE CONTACT PEST CONTROL FOR SERVICE AND EMAIL A COPY OF THE PEST CONTROL REPORT TO THE HEALTH DEPARTMENT WITHIN 10 DAYS.

The purpose of today's visit was to conduct a follow-up inspection. All critical items have been corrected. Discussed food employee training and cooler temperature monitoring logs for the 2DR Prep Cooler and 1DR True Cooler.

    Observation: The certified food manager could not provide a food temperature measuring device FOR (1) 2 DOOR TRUE PREP COOLER (2) MINI REFRIDGERATOR

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

Dishmachine: autochlor (chemical)
Discussed the following with the Certified Food Manager (CFM):
(1) Grease trap and hood are cleaned every month in the winter and every two weeks in the summer.
(2) Provide employee health training and keep signature pages on file. Complete within 10 days.
(3) All CFMs must have a CFM card with a photo. Complete within 10 days.
(4) Repair or replace 1 door True cooler and 2 door True prep cooler within 10 days.
(5) Repair handsink by the dishmachine area within 10 days.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. True 2dr preptop was discovered unplugged and CFM plugged in equipment and the air temp was at 43F and was still dropping at the end of inspection
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: walkin cooler, true 2dr preptop cooler, both artic air coolers, 1dr glass cooler, true 1dr cooler
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting board(s) along the cookline are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Food Temp Meas Device, Calibrated
    Observation: The food temperature measuring device was found out of calibration in the range of use.
    Correction: Food thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. It is suggested that food thermometers be calibrated at least once a week in a cup completely filled with ice and cold water to a temperature of 32°F or calibrated in a pot of boiling water at 212°F.

    Observation: Observed knives located at the cookline that are not being cleaned at least every 4 hours.
    Correction: Food contact surfaces of equipment and utensils must be cleaned and sanitized at least every 4 hours to prevent contamination by bacterial growth. CFM took knives to the dish machine
