  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

EHS provided additional training and written instructions on:
-How to test sanitizer chemical concentration
Good practice observed:
-demonstration of verification of digital thermometer
-Active managerial control: PATH in place which is line check every 15 minutes.
-equipment/food temperature log
*Please re-calibrate quaternary ammonium dispenser. It is at lower end of concentration level of 150 ppm according to manufacturer's directions.

02/20/2015Routine
The purpose of today's visit was to conduct a risk factor inspection. Observed acceptable levels of sanitizers in the sanitize basin of the three vat sink. NOTE: The dishmachine was not in use at the time of this inspection and technician has been scheduled for repair. Because the facility had proactively discontinued use of the dishmachine and had already utilized the three vat sink method of washing, rinsing and sanitizing utensils, there is no critical violation observed. Please fax or email a copy of the invoice for the repair of the chemical sanitizing dishmachine once the repair is completed this afternoon. Employee health policy in place. Observed several employees wash hands using proper technique. Foodworkers aware of their assigned duties and demonstrated knowledge of such. Thank you.

The purpose of today's visit was to conduct a routine inspection. Observed acceptable levels of sanitizers in the sanitize basin of the three vat sink and on the final rinse cycle of the Jackson ES2000 during today's visit. Observed several employees washing hands during inspection. Manager demonstrated knowledge of food safety principles during inspection. Employee health policy in place. Foodworkers aware of their assigned duties and demonstrated knowledge of such. Thermometer calibrated and used by the certified food manager. Thank you.

The purpose of this visit is a risk factor assessment. Quaternary ammonium in the wiping cloth bucket and sanitize basin of the three vat sink both tested at 200ppm. Chlorine in dishmachine final rinse tested at 100ppm chlorine. Thermometer calibrated and measured 32f in ice bath. Employee health policy in place. *Remember to remind employees to wash hands by first wetting hands with warm water, lather with soap, rinse and turn off faucet with paper towel and to use 20 seconds time to properly wash hands. The majority of the time should be on the lathering and scrubbing step. Thank you.
  • Critical: Hands and Arms / Cleaning Procedure (corrected on site)
    Observation: Observed food employees using improper handwashing procedures. Observed an employee wash hands not lathering with soap and with less than 5 seconds total time at the handsink.
    Correction: Food employees shall wash their hands and exposed portions of their arms by: 1) discarding used gloves
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink at the front handsink near the steam table being used to store utensils.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.

Observed several employees practicing proper handwashing and re-gloving.

  • Equipment / Good Repair / Operation
    Observation: The quaternary ammonium dispensing unit is dispensing a hot water mix between 126f-129f and water temperature was unable to be adjusted during inspection. A service order was placed to the technician for adjustment.
    Correction: Repair/adjust the unit so it dispensed/mixes water with the sanitizer at the appropriate temperature and strength. A follow-up will be conducted with in 10 business days.
  • Dressing Areas and Lockers Designated & Required for Employee Clothing Changes (corrected on site)
    Observation: Dressing rooms/areas are not designated for employees who routinely change their clothes in the establishment. Observed a usb charger on a sealed wooden prep table in the back.
    Correction: Provide separate dressing rooms for employees who routinely change their clothes at work to prevent contamination of food, food equipment, and food contact surfaces.
  • Critical: Conditions of Use/Presence & Use of Toxics Per Law (repeated violation)
    Observation: The Quaternary ammonium sanitizer at the three vat sink is dispensing at 126f and tests above 400ppm. Manufacturer directions are to use 65f-75f water and may be dispensed and used at 150ppm-400ppm.
    Correction: Poisonous and toxic materials shall be used according to law and per the manufacturer's directions to prevent injury and contamination.
