PREOPERATIONAL FINAL INSPECTION 1. Plans were revised after being approved by the Health Department and the new revisions were never submitted to the Health Department for review and approval before construction. Major equipment changes were made and some which were added were never on the original approved plans. Therefore, the applicant shall submit the revised plans to the Health department by no later than December 2, 2013. 2. The number of the three-compartment sink were increased in the new revised plans from 1 to 2, and the mechanical dish washing machine was upgraded from the one that uses 55.5 gallons per hour of hot water to one that uses 68 gallons per hour of hot water. THE APPLICANT HAS DECIDED TO ELIMINATE OF OF THE THREE-COMPARTMENT SINK DURING THIS INSPECTION UNTIL THEY CAN DECIDE WHETHER OR NOT THEY UPGRADE WATER HEATING EQUIPMENT. 3. Smooth, nonabsorbent and washable ceiling finishes have not been installed at the dry goods storage and at the two rest rooms. *Approval for a Health Department permit is hereby granted contingent on the correction of the above listed items. This inspection report shall serve as your permit until official permit is delivered to you. No equipment changes/replacements/additions are allowed without approval by Health Department.
