Does anyone have experience with travel health insurance?

Last December a friend had to fly to St. Kitts because his brother had a stroke while on a cruise. They brought him to a hospital that was clean and gave him 3 meals a day and that was about it- no TV, no radio, no Wi-Fi, no towels (family is supposed to bring them!) and no physical therapy. He could not have made it home alone- needed a wheel chair and is still having problems with reading 8 months later. NOTHING related to COVID-19 will be covered.

I had travel insurance (broader than just health coverage) when I went to South America in March and was hoping that it would cover a flight I'd booked to go home early after the tour was cut short. I was going to fly home through Colombia but they closed their borders, even to transiting passengers, after I booked the flights. Nope. Government actions not covered.
