  • Critical. Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit. PH dressings in salad make table at 46 Degrees F. 47 Degrees F for pasta salads at frontline display reachin cooler. Corrected On Site, moved to freezer for immediate cooldown.
  • Critical. No conspicuously located thermometer in all cold holding units.
  • Critical. No quatenary ammonia test strips when using quatenary ammonia sanitizer at three-compartment sink, or for wiping cloths.
  • Observed sanitizing solution for wiping cloths not free of food debris and visible soil.
  • Critical. Observed interior of microwave soiled.
  • Critical. Observed handwash sink used for purposes other than handwashing. Utensils stored in rear kitchen handwash sink.
  • Observed dirty mop water stored in mop bucket, and wet mop not hung to dry.
  • Critical. Manager lacking proof of Food Manager Certification. NOTE: MUST HAVE CERTIFICATION BY 09/24/09.

  • Critical. No chlorine test strips available, will be using chlorine sanitizer at three-compartment sink and for wiping cloths.
  • Plumbing system improperly repaired. Cold water faucet handle no fully secured at frontline handwash sink, must be repaired. Cold water (and hot) is available.
  • Critical. Portable fire extinguisher not properly mounted at least 4 inches off the floor and the top no higher than 5 feet off the floor. For reporting purposes only.
  • Critical. Manager lacking proof of Food Manager Certification. 60 days to comply.
  • Critical. No proof of required employee training provided. All public food service establishments must provide the division with proof of employee training upon request, including, but not limited to, at the time of any division inspection of the establishment. 60 days to comply.
