Did Australia and the UK stop issuing altered certificates, or do they just make the originals available once the children have reached their majority?
In the UK the adoptive parents are issued with a Certificate of Adoption they don't alter the information on the adoptees Birth Certificate

I'll look that up here's what it looks like:http://www.ukcertificates.com/images...land-wales.jpg
An adoptee uses this for legal purposes. It carries truthful information (ie it does not state that adoptive parents gave birth to the adoptee the way US 'amended Birth Certficates' do)

Adoption and Children Act 2002 (scroll down to section 77) http://www.legislation.gov.uk/ukpga/2002/38/section/77
