Default Recommendation needed: Seamstress to make a custom slipcover for an armchair

The beautiful glider armchair that we purchased for our son's bedroom five years ago is now looking ratty and worn. Its amazing how young kids can turn any piece of furniture into a jungle gym!

In any event, it will cost more to have the furniture re-upholstered than to buy a replacement chair. So we thought that a custom-made slip cover would be a good solution.

Any recommendations for a seamstress/tailor that would be able to make a custom slipcover. Or any other suggestions for how we might cover up this eyesore?
