Maybe you'd rather live in Panama instead of Guatemala, but the fact Panama feels more vibrant and cosmopolitan doesn't change the fact that about 3 times as many people live in the Guatemala City area than in Panama City.
52 times more people live in Guadalajara, Mexico than in Georgetown, Cayman Islands; yet the second one (a gamma +) is much more relevant to the world than the first one (gamma).

I am surprised that Quito ranks higher than Guayaquil. Although Quito is the capital of Ecuador, Guayaquil is the business center of the country (kind of like comparing NYC to Washington, D.C.).

Like many others, I'm surprised Panama city doesn't rank higher. Perhaps that city has been claiming more recent attention, but it isn't as powerful/influential regionally (outside the canal) than I thought?
