I know I'm really late to answer this, but I just did this a few months ago, and all I did was take my old permits to DMV, and they gave me a form there that required a minute or less to complete, and then they traded my permit for AZ ones. You do have to bring all of your old permits - can't just bring one, and get 2 new ones. Also, if you're from a state that prints the placard #s on the DL, that's not good enough either - need the blue hangtags. But if you have those, it's quick & easy (or, as quick as any wait in a DMV is).

Illegal use of Handicap Tags is one of the most enforced regulations. All tags have a number which Parking Contro and Traffic officers can check in seconds. The reality is Handicap stickers are checked about 80% of the time (National Traffice Safety Study) by Patroling officers, with individuals using a family members tag when the family member is not in the vehicle being the most frequitly cited.

No. From the MVD website:

Placards are issued to the applicant and may not be transferred to, or used by, another person. Plates are assigned to a vehicle. You must notify us prior to transfer of the plate to any other vehicle that you own or lease. Only one valid placard is issued per person. The placard may not be duplicated.

You can get one plate & one placard. Put the plate on the car u drive most & that IS REGISTERED IN YOUR NAME. Put the placard in the other car.

I did this and was in and out in 15 minutes traded my Fla Placard 1 day before expiration for a new 5 year Az placard. I used my Florida placard for 4 years here without an issue. I think they are recognized in all states because if you have an out of state visitor bring one with him to another state int would only make sense it would be acceptable.

I think I downloaded form up, had local doc sign it. waited until my WA plates were do to expire & went in got my AZ drivers lic, AZ disable plate & placard all within 15 minutes. very easy

I don't know with certainty, but I would think you do. A physical disability which requires a handicapped parking space is not synonymous with an inability to pay for parking.
