I've had people in the rightmost lane almost come into the left lane multiple times as well. Last time my husband honked as some woman almost completely slammed into our car by crossing into our lane and she looked at us angrily---like he did something wrong.
I think people are very bad at gauging when they are turning into multiple lanes. I am very cautious when I am turning into multiple lanes because I do NOT want to be hit since "almost" has happened multiple times---sadly I don't think many people turn cautiously---they just turn and veer out.
I see it every day. Here it's mostly drivers trying to get in front of others. They know full well that they are going to want the right lane after the turn, but they don't want to be behind one or two cars, so they get in t he innermost lane and plan to cut over during the turn. I usually neutralize their plan by making the turn faster than they do.

This is what I experience more often as well. At one double-left hand turn I frequent, I use the leftmost turn lane as I have to make another left turn soon after. All too often the person in the turn lane to my right just wanders over into my lane mid-turn, where I have nowhere to go. I now have one hand hovering on above the horn at all times at that intersection. They have no excuse; the left hand turn signal has a sign above it that illustrates it being two separate left hand turn lanes that stay separate throughout the turn. Grr.
I had this happen this week actually! I was in the middle lane and a moped was next to me in the left lane. I'm always pretty aware of drifters through this intersection but the moped decides to go STRAIGHT and nearly smacks into the side of my turning car. Good thing those bikes seem pretty agile and no one was around because he was able to totter off behind me. No helmet either, could've been worse.
