I want to design a decal for my car. What kind of company would I send it to to have it made? Do they last a long time without fading? How are they applied? Are they adhesive-backed?
In some of the design work that we do, our clients have asked to have that transferred to their vehicles. There are a few options for you to think about - adhesive and magnets. Call up your local sign store for this.
They can last a very long time depending on the quality of the material used for the decal, adhesive, weather conditions, colors, etc.

I have a greyhound magnet decal on the back of the 4-runner it has been on for about 3 or 4 years
I would also check with a sign shop that makes these

I have found companies on ebay offering these services. With the ability to eMail your design it's easy to send it virtually anywhere.
Yeah its easy to send your design, and relatively cheap too. My friend runs a sign company and decals cars. The only problem with DIY jobs is that its not as easy to place decals in vehicles as people think. If you make a mistake by hanging it crooked its kinda tough luck.

I should have mentioned that you can check and see if there's a Craig's List serving your area. They have a section for artists and you could post a request for what you're looking for. You might even find a local artist who would do it in paint for reasonable $.
A sign shop is your best bet. We used a few different ones in our area for sports decals for teams. Depending on how many you want and if they already have a design in their system will determine the cost. I had just 1 done for something a few years ago and it ran me about $15 (gosh, wish I could remember, darn old age) for a small 3 color design. Most are stickers that are best put on the windows and like mentioned before, if you put it on crooked there is nothing you can do about it. The other thing that I have seen for some are the window clings, easy on easy off, just make sure you take it off BEFORE you go thru the car wash. I had a magnet one time and it got stolen. The window cling is less likely to get stolen as most will think it is a sticker and I actually like these better as you can take them off easily when you wash the car and there will be no evidence of a sticker of any kind having been there.
