Having way too much time on my hands, I decided to look this up. I have always admired the players who do a little bit of everything on the court. Numbers are from the 2017-18 regular season.

Jimmy Butler barely missed out with 22.2, 4.9, and 5.3. So that is 13 players out of the 450 or so listed on NBA training camp rosters, or 2.9 percent of players--pretty rarefied air. Westbrook, LBJ, and Harden are the kings here. If you add the three columns, Westbrook is 45.7, Lebron 45.2, and Harden 44.6.

Golden State, having added Cousins, has 4 of the 13, and Houston has 2 of the 13, so that is almost half of the list on just two teams. Harden is the only shooting guard on the list, although he is really more of a combo guard. Lebron the only small forward, unless you count Durant as a SF.
