The starting on a shoestring part becomes the hurdle. As others have pointed out, regulations, especially regarding seniors, can involve a large hurdle. Before you open your doors, you should talk to an attorney (I'm not one, I usually try to avoid them, but that's the reality). Next, as also pointed out, you need hired help since you may not be able to do this alone. Even if you do not, you will need to establish some sort of legal structure (LLC, S-Corp, etc.) and set up a bookkeeping system.
An option may be to set yourself up as a subcontractor. You could then go out to existing businesses doing this kind of work and see if they have any ancillary services that you could do for them. Being an RN will give you the credibility and entree. In other words, you could fill in some of the voids that they may have in their operation with your expertise and start doing it on less of a budget.
