Question Help With Form

Hello,

I am filling out a form that is similar to this form. I am ordering items in bulk from an online wholesaler, and I'm confused on a few questions.

Percent of Discount from Catalog or Published Price List:

Does this mean that I give a discount on the items that I have or give the discount provided to me from the wholesaler or something else?

Percent of Discount from Posted (Shelf) Retail Price:

I don't know what this refers to.

Also on the form it states that I must provide a catalog or price list. Is this a list I create or a list I get from my wholesaler?

Thanks

Generally, this is used when you have a sales rep or a major account. The rep gives you a discount off the catalog list to keep you as a customer. Someone walking in off the street who gets the catalog sees only the inflated price. There are other possibilities as well. If you order one widget, you might pay the cat list price. If you order a truck full of widgets, you call your rep first and ask what the best price is that he can give on that truckload. If you don't get a discount, you search other suppliers.
