I blatantly ran a red in a photo enforced intersection back in dec 15 because I spilled piping hot coffee all over myself. I was in a rental car, so I have been monitoring my credit card since Avis typically pleads guilty and charges the driver for the offense.

Is 45 days enough or is the California Traffic Violations Mafia (or CTVM) slow and inefficient to the point where I could get a ticket in a year?

sounds like youre in the clear

they usually mail those things out within like a day

As you spilled the coffee did you look down? The moment the camera takes the photo of the front of the car I think a 'clear' image of your face is needed. If you're looking down then the police wouldn't have the evidence connecting you to the car.

That doesn't make sense. Especially when it's a rental car. They know who drove or actually was in charge of the vehicle at that given time. So the rental company would get the ticket and they would probably try to get the charge back from the renter. Maybe you're such a good customer that they waived the fee for you :-)
