Hi,

I need to get a companion animal prescription for my mother. She needs it, carries it everywhere, and we need the prescription to help her with her housing needs now. Can you recommend someone who can help her?

Her doctor and/or psychiatrist.

It seems rather obvious that her doctor would be the one to give the prescription. Are you perhaps talking about a certification to show that the animal is a service animal? Or that you are looking to certify the animal? Google charlotte nc service animal certification. There are a few.

Revised ADA Requirements: Service Animals
You may have a tough time passing a companion pet off as a requirement under ADA for her housing. Hopefully she has an understanding landlord or living arrangement. I wish you the best of luck.
