House of Pizza on Central? It's a hole in the wall and they serve pizza and subs. But I think the owners are Greek.

Mario's beside the gym (which was a movie theater 20 yrs ago) in Matthews Festival off Hwy 74.
Family owned - great pizza and calzones.
Frankly it is tired on the inside, but the food is good - fits 'hole in the wall' request.
Avanti in the Arboretum is also family owned - good pizza & pasta (love their baked ravioli).
Recent renovation, very clean.

Is the Open Kitchen still well... open? If so, it has been there since the 1950s. It's pretty hole in the wall looking. It's on W. Morehead.

Heck yeah. Bit of a rough area nowadays, but they make one of the biggest and best meatball parm. subs I've ever had. I love that place.

Open Kitchen (http://www.theopenkitchen.net/ - broken link) is probably the most "whole in the wall" type of place I've ever been too...and it's not bad food either! Yes, they're still open. They're a Greek-Italian place (like many are) and offer a sweet tomato sauce (I say that because some prefer a traditional savory sauce, calling sweet fake). Lots of history and friendly staff.
I don't particularly have a favorite spot in town, but I do have regular places:
- DaVinci's in the University City/Davis Lakes area...NY style pizza, good and decent prices
- Villa Francesca in Uptown at the corner of 7th and Caldwell..."Grandma's Special" pizza is one of the best unique pizzas I've ever had
