So here's the problem. My husband has never had a copy of his birth certificate. He is 44 years old. His mother recently told him that when he was born she lied on his birth certificate and put his name as Carl Lee Jones and her name as Carla Jones. His real name is Carl Lee Evans and her name is Dianne Jackson Evans. His fathers name is Jerry Lee Evans but he is now deceased. Where do i start?

His real name is the name on the birth certificate. What name is on his SS card, driver license, state ID, passport, etc?

Does he have a drivers license? If so, I'm pretty sure he had to provide it to DMV as proof of his age and person.

Go to the health department in your county for birth and death certificates, at least that's how it is done here. they will give you a form to fill out and you put down as much info as you can. if more info is needed they will ask you questions and tell you what you need to do if it doesn't goes through. I have done this for older family members and it took me only about 30 minutes and 12 bucks.

A few years ago I ordered my birth certificate and that of my then-spouse on line. Maybe your state allows that?
Try looking at the vital records section under the state site. Use the name on his driver's license. If that doesn't work, use the name you were told instead.

All of his ID has Carl Lee Evans, drivers license, ss card, and even his military id. He is trying to get a passport but they require a birth certificate. He was able to join the military only because the gulf war started when he joined and they just pushed his paperwork through and never requested it.

If he was born in Cook County you will find all the info you need here:

Vital Records

Start with the name on his ss card.

It is expensive to request this online. It will be much cheaper if you can visit the office in person. I always went downtown since I worked there. Really simple. You might need to bring proof of your marriage to get his certificate because they don't give those out to just anybody.

Ok so the problem we run into is that when we go to order the birth certificate online we put the name thats on the birth certificate (Carl L Jones) and when we get to a point where they ask for his ss#, but his ss# is listed with the name Carl Lee Evans and it gets rejected. They can't understand that carl jones is carl evans. We don't live in Chicago now we live in Virginia. As far as my husband can recall when he was in high school he went to get his ss# and all he had to provide was his school records, that had carl evans as his name. He didn't even know that he was born carl jones until he was joining the army and asked his parents for his birth certificate, thats when his mother told him the long story why she did this. I may have to hire a lawyer to resolve this.

Okay, but how did he originally get a state ID and SS with no BC? I've just never heard of that happening (different name on BC than SS) without a legal name change. The BC is the creme de la creme of ID's because you need that to get the other forms.
