I believe the lists from the website I posted are more recent, but I'm just not sure exactly how recent its is. According to that website, Rhode Island is 19% Italian, which is #1 among the states and the Providence area is about the same percentage.

Note the OP said highest number of attractive Italian girls. Most people have been responding to places with the most Italians (really Italian-Americans). Not quite the same thing. How do we do a better comparison?

How about Buffalo, NY....they seem to have a few nice looking Italian girls.
Or if the OP would also consider Canada...Toronto is known to have some nice ones too

Well, there's really no way to tell, partly because attractiveness is subjective. But would you not be more likely to find more attractive Italian-Americans in the places where Italian-Americans are mostly found in? For example, looking for an attractive Italian-American woman in New York City would probably be easier than doing so in Phoenix, AZ. Logically, the places with the most Italians would likely also have the most attractive ones, easiest to find as well, city or state.

Are there attractive Italian-Americans all throughout the US? Sure, but the group is concentrated in specific areas of the country, the Northeast really, so I'd start there for a search.

Well, yea. I was [half] joking. My high school was about a third Italian-American, maybe I should look through photos of the girls.

My town is about ¼ Italian-American, I should do the same.

Surprised Rochester and Pittsburgh have such high percents. Most likely since they are less diverse of areas.
Off the forum topic, but South American metro dwarf any American metro in terms Italian numbers and %.

São Paulo 6,000,000 47.6%
Rio de Janiero 600,000

Argentina is roughly 60% Italian, Buenos Aires likely higher
