Pittsburgh State (KS) Gorillas. I think it's fine for football, but kind of unbecoming for their women's sports teams, IMO.

The Rhode Island School of Design has the nickname "Nads". Their mascot is a giant penis named Scrottie. I'm not making this up! There are youtube videos of the giant penis skating around the ice rink with lots of children attending the game. Fans yell "Go Nads!"
