Only when I build a new PC or a new version of Windows comes out. I ran the same install of XP for 5 years, then built a new PC for Vista, ran that for 2.5 years until Win7 came out, then formatting and installed Win7 a couple of years ago.

For my Windows computers, I find about every 2 years its good to just reinstall everything and get the computer working better again. For my Macs, never, but when I replace one, I usually install everything clean rather than use the migration app.
Windows98: every 6 months like clockwork.
Windows XP pre-SP3: every year like clockwork.
Windows XP post-SP3: Pretty much haven't yet, although I don't really use that PC anymore.
Windows Vista: Haven't yet on my laptop (2 years)
Windows 7: don't plan too unless I feel it needs it.
Everyone has their own approach when it comes to deciding when to reinstall an operating system. Some people prefer a structured time frame such as every six months to ensure an uncluttered operating system that will run quickly and feel “fresh”. Others prefer a more long term approach such as once per year or longer since they rarely have much trouble from their systems.
