Since there may not be job positions near my home, should I enter
1) The distance I am willing to commute (20 - 60 miles one way)
2) My willingness to relocate at my own expense
3) My willingness to travel (0 - 100%)

Shane has good info. When I tried to find a position in another state, they could care less what I had to say. Sorry to say, I don't think they took me serious due to my location. Now if you have an address of a friend or relative in the area, use that address. However, if you get called in for an interview, be prepared. Myself, it never happened.
Are you in a high demand field? Then it may not matter. Computer guys like me are a dime a dozen. However I see many positions I get passed over and they advertise again in a couple months. Not all hiring agents know their stuff either....
