Default Filing a lawsuit in Jefferson County

I want to sue someone for about $6,000 who now lives in Utah. I put together a Small Claims suit, but since he no longer lives in Jefferson County (where the incident occurred) I have to go a different route.
Does anyone know where I can find the paperwork to put together this kind of suit? I think it's called a County Court Filing or Summons Complaint? I'm not sure on the legal terms.

This is for Jefferson County, Colorado.

You need to seek a lawyer's advice. It is incorrect to believe you cannot sue in Jeffco just because said Defendant lives in Utah now. There are rules as to venue, and without typing out a long lecture, you aren't going to learn them overnight.

It sounds to me like you are confused as to small claims court vs. county court. The only difference here is the amount of money being sued over. I cannot recall the max. allowed in small claims, and any amount can go to county court. Although be aware that if you try to represent yourself in county, your opponent can easily bring in a lawyer, whereas small claims court is for the people, and does not allow lawyers in their court.

See a lawyer.
