I understand if this is not allowed. If that is the case, accept my apologies ahead of time and just delete. My husband is taking a class and one of the requirements is that he complete a health survey. He has chosen to do it on diabetes. It is a short surveymonkey ten question survey. If anyone would be willing to complete it, it would be greatly appreciated.

In other words, he needs to survey a group of diabetics for his class. Maybe he's training to be a diabetes educator, or something like that.

Diabetic Health and Nutrition Survey

Part of the class is getting data regarding health related issues. The entire rest of the class is comprised of women. They are doing breast cancer since it is Breast Cancer Awareness Month. However, his professor would not let him survey women about breast health. Since this project is 30% of this class, she said he had to find another health related issue and do a survey on that. He has chosen diabetes. Since he is not a member of City-Data and I am; I figured I would help him out by posting the survey. He will then collect the data from the online survey and the people who he has handed paper copies of the survey and write the rest of the project from there.

I am sorry if my original post was confusion. That's what I get trying to post way past my bedtime.

Yes, he is in a nursing program and it is a class on Global Health
