Could something have scared him upstairs? Are the stairs non-slip or slippery and what about the floor upstairs. We have a lab that gets afraid on slippery surfaces.
Glad he is OK! Just saw this thread and my first 2 thoughts were (1) blood clot or (2) hip dysplasia. Sudden behavioral changes always get my "it could be something bad" radar going.
Since no medical issues (and I presume eyesight was checked?), then it obviously is behavioral and probably the result of some "scary" event (scary to the puppy). So many times these behavior-changing events happen outside our knowledge or we have been there, but the event didn't seem like a big deal to us.
The few times you've gotten him upstairs, how was he coming down? Still hesitant? And when upstairs was he nervous or did he appear stressed? Trying to whittle it down to exactly what is bothering him!
Bare stairs or carpeted? Open stairway (such as railings on one side or the other or just treads and no risers)? "Cave-like" (full walls on both sides)? How well lighted is the stairway or is it somewhat dark? And on that topic, is it at any time of day or night he won't go up the stairs or just at night?
First occurrence was last Tuesday night ... what type of weather did you have? any storm? OR being on the tail-end of the New Years holiday, were there any fireworks?
