Years ago I used a Pre-Algebra book by Holt, black with a picture of the Space Shuttle on it, I think. It broke every operation down into small steps and provided clear explanations and a lot of practice. It would have been new in the middle to late 80s. I don't know what they have done to their Pre-algebra texts since then, but I know they are all entirely too big and heavy. There is no need for a math book to have that much extraneous information.
