As a nurse,once someone is diagnosed with a chronic dusease/mental issue it stays.
An IEP needs to be reviewed and updated (at least) every year, but that has nothing to do with being re-diagnosed. Any chance your cousin's daughter's school is talking about the annual IEP review that's required under federal law?

Two year old IEP paperwork would put them out of compliance, because it's not an active IEP (only good for a year). But not because of the disability diagnosis. Old/noncurrent IEPs may need additional evaluations done to appropriately update the document, but those evaluations would be for educational purposes, not diagnostic ones.

Her daughter does not even have an IEP.

She is still trying to get her daughter evaluated by the school. I think the school is refusing the medical diagnosis made by the psychologist at CHOP.
2017 will be going on year 3. The poor kid cannot write not do math,and is being in everything.

While teaching special education, I've never personally experienced a school district rejecting a legitimate diagnosis from a licensed diagnostician. I couldn't tell you why the school is disputing the diagnosis without more information.

Has she put the request in writing to the school to have her daughter evaluated?

The second link is the IEP process and a child's rights by law. It was written by a team of lawyers at Goodwin Proctor. Worth taking a look.

I'm delving into this whole process though I am in NC. I put the request in for the evaluation and it took about 4 weeks but I had a meeting with the head EC teacher, Principal and my child's teacher.
