Perhaps her/she is traveling in a budget. Two years ago I visited Italy with very little money. Although I was able to try the local cuisine -not amused by Naples pizza- most of my dinners were at Micky D's. Affordable, tasty and familiar.
McDonalds is more expensive than Pizzas here.
Not liking Naples Pizza (which cost like.... 3-5 euros for a 50 cm wide pizza) is puzzling to me to say the least.

Yes the ketchup is free and don't worry there are McDonald's everywhere in Europe and they are always packed. Europeans love their McDonald's and McCafe's
Right. I've haven't seen any of those. Not that I've been at McD's yet this year.
