Those areas have most of the city's museum, the gothic district, has a major theatre/opera house, etc. Also a lot of it's buildings have been converted to hotels or to Airbnb apartments.
This has been true of other cities. Times Square was a very rowdy places, but all the hotels were nearby and it continued to gentrify until the gentrification became complete, with the exception of all the homeless you see around Port Authority.
So you will have the same thing with the Rambla area in Barcelona.
And please, it's not like people are going to faint just because they saw a hooker or an addict.
