This happened today where I work out and I've heard this goes on at other gyms. People standing right outside the door trying to finish their cigarette before coming in to work out. Another person finished her workout and as she left the locker room, she had her cigarette in her mouth and lighter in hand and lit up right outside the door.
Ive never seen or heard of this before. Heck, I almost never see cigarette smokers anywhere near the gym. Come to think of it, I rarely ever see people who smoke, except for may be the older middle eastern folks.
I've never seen that at any gym I've gone to. What is a problem at my gym is the old guys who get naked and aimlessly wander around the locker rooms. The gym actually had to put up signs because they were getting so many complaints.

i don't see how that would affect you in any significant way
I've never seen it either. But I have always lived in an areas where it is unlawful to smoke within a set distance from a building. Thank goodness for anti-smoking laws! I don't want to breath that crap.
Guys shaving, brushing teeth or just standing around chatting while naked in the locker room. You get dressed first, then do whatever you need to do to get ready for work.
I use an e-cigarette now but I smoked tobacco cigarettes for many years. And I'd smoke them on the way to the gym and light one up right after my workout.
Nicotine addiction is VERY hard to get over.
As long as people aren't blowing smoke into your face, don't worry about it.
And that's why I quit working out at the gym and just work out at home now
