Baseball caps
Hair color/highlights
Tight shirts (yes, even if you've got the perfect physique I don't want to see it)
Hunting gear/camo-looking stuff
Anything that really, really looks like it came from the women's department
There's probably more I'll remember later.

It's probably incredibly shallow of me to feel this way, but I am appalled by guys who still wear those stupid pants that hang down halfway over their butts, AND by guys who wear "skinny" (girl) pants.
It's kind of like a "first impression" thing. When I see guys dressed like that, there are two words which come to mind..... weirdo and idiot.
Give me a guy in a pair of nice-fitting, old-fashioned Levi's or Carharts and a flannel or hickory shirt and I'm a happy camper.

If I have to think of another one, baseball caps. Boring and unbecoming. They serve a purpose on baseball players.

LOL..... Baseball hats I don't mind...unless they're turned sideways or backward!

shorts so long they might as well be pants
shorts so short you look like you're a gay 80s aerobics instructor
white socks worn up to the knee with shorts (are you a baseball player?)
any of that Tommy Bahama stuff (unless you're retiring to the tropics)
boring colors - there are colors outside of neutrals
Men over the age of 25 or so still wearing skater & surfer clothes or hoodie jackets for casual unless they are partaking in a skating, surfing or athletic kind of activity
athletic wear worn as casual wear - put on some real clothing!
t-shirts with logos and/or dumb sayings
Ed Hardy stuff & anything similar to it - ugly, tacky graphic tees, basically
True Religion jeans with their hideous pockets
those "outdoorsy" looking kind of boots (don't know if there's a name for them), often with a really chunky sole
cargo shorts or anything camo print
The biggest complaint I have is ILL FIT. Too many men wear too baggy clothing, including their suits (get a tailor!!!!!!!!), or there are the young hipster types who wear way too tight (no one wants to see the outline of your balls).

Wow....and I thought I was picky!

A few times I've seen young attractive men wearing Crocs. It's not really that horrible, but just kind of puzzling because I think of female middle aged nurses or gardeners when I see those shoes.
JNCO jeans and shorts used to be really popular when I was in middle school. I'm kind of glad that's over now.

OMG! I used to wear them! HAHAHA!@ GUILTY but of course I was a child.
The opning in the bottom comes in different size 17", 20", 24" and 30"!

It's not a huge deal, but I'm not a big fan of white socks with everything. For athletic purposes with gym shoes, sure, but if he's wearing dark jeans/pants with dark shoes, he should be wearing dark or matching socks.

Flip flops (should only be worn to/from the pool, in the locker room, or at the beach. besides, they're horrid for your feet!)
Ed Hardy t-shirts.
Baseball caps as a fashion statement. (I don't mind 'em if they're appropriate to the activity - at the beach, when playing a sport, etc)
Lewd/Crude t-shirts. (Thankfully, I don't see this very often.)

Denim shorts that go past the knee.
Sandals and socks.
Skinny jeans.
My BIL wears denim shorts, sandals and socks, and pastel guayabera shirts all the time. He just turned 40 but he dresses like he's 80.
I do think flip flops are reasonable though when you live somewhere really hot.
