if im at work, i dont button the top button of any dress shirt unless im wearing a tie. Outside of work I follow that rule 99% of the time, but certain shirts with certain outfits I'll button the top button.

depends if you are hipster.

hipsters always button the top button without a tie. so depends what look you are aiming to achieve.

Depends on the dress shirt. Some look better with the top button buttoned and some look better without. It also depends on how conservative you want to be. But with a tie definitely buttoned.
