Not always indicative of smoking - there are autoimmune diseases which can cause the mouth to "draw", even in non-smokers. People who have had scleroderma for several years often develop this trait, and it has nothing at all to do with smoking or not smoking.

There is an actual medical term called "smoker's face" - this term has been around since 1985, used by doctors.

Here is the "twin study" from webmd showing the differences in non-smoker vs smoker face:

There are just too many people with access to photoshop or similar programs to alter photos. It is just too difficult to identify smokers based on pictures. But I'd say I can sucessfully identify a smoker in real life just by looking at them. I'd say I can do this with about 98% accuracy.

Not really. You can usually tell heavy smokers who have had a long habit. I've been on and off, more off and mostly light (pack or two a week). Most people never realized I was. If you got close, you could still smell it. I never had stains on my fingers or teeth, although the teeth aren't particularly white as I don't bleach them and just don't have particularly white teeth. I have a porcelain crown which doesn't really stain past what your regular dental cleanings will remove from the surface from when I was a teenager. It's the same color as the rest of my teeth. What I do tend to get is psoriasis. So I guess if you looked at my feet you could tell if I'd been smoking.

Shortness of breath, smell, always have to take a smoke break, these are things not noticeable in a photograph. Wrinkly skin, yellow teeth these are things noticeable in photographs, but smoking is not the only source of these. So the answer is no, not unless you know the person.

Not always, but a lot of the time you can tell be the facial-skin if someone is a smoker.

Yes, that nasty habit reeks havoc on your skin.
