Many of those places on the list are very misleading. For example, Treasure County, Montana, has one, because it has three exits from a barren interstate highway, and might be the only food stop for 50 miles in either direction. But the county has less than 1,000 people. Refugio, Texas, is on a stretch of high speed 4-lane highway and in a run of 200 miles, it's the only town that the highway actually goes through, instead of bypassing, and it has traffic lights and stop signs, so of course it has a lot of fast food. But people who live there eat very little of it.

The counties with high density of fast food places are nearly all lightly populated counties with interstate highway exits in them, and do not reflect local consumption.
