I sent a dish back once that had a bunch of unopened clams in it. It came back with the clams open, but pieces of broken shell shards inside the clams. Manager got called to table and we had a discussion about serving clams that didn't open in the first place, then forcing them open and serving again. Dish got sent back with no replacement and taken off check. We never went back.
This has happened to me. The "chef" came out of the kitchen to tell me unopened clams are still safe to eat.

I had cooked spoilt fish served to me. I had the waitress try a bit of it and the look on her face was priceless. Since there was plenty of other food (as it was a full dinner), the restaurant agreed for me not to pay for the dish as I had no interest in ordering something else or more of the same.
Did you tell the Chef to eat them there in front of you ???? With his TRUMp LIKE attitude it was time to BULLY him out of the place.
I was so annoyed so I very politely told him what he could do with his "safe" clams. Then I blasted them on Yelp, FaceBook and Instagram.
What burned me the most was they left the entree on the bill at full price.
