Pretty gross, I'll admit. I have never made anything that extreme, close, but.....
Once, my son was in a particularily bad humor right before his birthday. You know that song: Nobody likes me, everybody hates me, guess I'll go eat worms? That must have been what he was thinking about cause that's what he asked for. So I baked the layers, then carefully sculpted out holes with an ice pick. I threaded gummi worms through the holes and iced it. Well, his grandmother who was in denial about honest feelings, was quite upset because it wasn't all butterflies and bluebirds, but everyone else thought it was funny. Poor women, she never did understand that people need to be careful what they wish for around me, but I'll give it to them.
Another time, my niece graduated from Huron High in Ann Arbor. The high school sits next to the Huron River and the school mascot is known as a Huron River Rat. You guessed it. I baked a 1/2 sheet cake, beautifully decorated in her favorite flowers and school colors. When I was all done, using a fork with wide tines to simulate teeth marks, I 'gnawed' off one corner, made footprints in the frosting and placed a gummi rat I had saved from the previous Halloween so he appeared to be savoring his meal. Everyone was surprised, my niece was delighted that her cake was so unique compared to the ones her friends got.
I would if it tasted good. Someone once made me a birthday cake that looked like two huge, pink butt cheeks complete with pimples and... er... brown stains.
