It happens to me about once a year, and my wife is almost always there to witness it. I've also slipped on a banana peel and gotten my hand stuck in a pickle jar (had to break the jar). Fortunately I have not had a piano or anvil fall on me.

I was getting back into my car at a hotel when I stepped in a gooey wad of gum, and while I was hopping around trying to figure out how to scrape it off, I tripped and fell on the curb and sat in a pile of warm dog poop.

I actually did that. I stepped on the hoe and it shot up and hit me in the forehead... hard. I cracked up at the stupidity and then looked around to see if any of my neighbors saw it.

They didn't.. thank goodness.
