I use grand rather than great. It seems wrong to just skip over grand, when grandmother and grandfather don't.

thanks for the replies, folks!

I have been calling the 18 yr old "my niece" out of simplicity, but her mother is really my niece. We're so close in age, that she's more like "my little sister" emotionally. (I was the last child of my parents).

I call my nieces' kids my great-nieces (and one great-nephew). We never used the "grand-aunt/uncle" term in my family. I don't know anyone who does, actually.

But great-great is used for grandparents. My great-nephew was my grandmother's great-great-grandson, born on her 90th birthday. We have a picture of her holding him.

Now he is almost 20, and if Mom hangs on a few more years, maybe he'll make HER a great-great!
