Mass transit is this area's biggest need, starting with an efficient bus and light rail system in Birmingham proper. As for the roads the completion of I-22 is on the top of the list, finding a long term solution for the US 280 rush hour gridlock.
Two perennially snarled interchanges where I-83 exits to a single lane to rejoin I-83 - it does that twice in ten miles. To add insult to injury PA 283 does the same thing as it emerges from I-283.
Another idea would be an extension of PA 581 north under Blue Mountain and through Cove Mountain to take the international US 15 traffic. That would be a winner even with a $10 EZPass-only toll.
Maybe even better than the I-83 improvements would be a new highway from the I-78/81 split passing Lebanon and a new I-76/PA Turnpike interchange crossing the Susquehanna at Three Mile Island, tying into I-83 north of York, continuing to Hanover, Frederick, Harpers Ferry, and Front Royal.
Yep, the biggest infrastructure need in the Harrisburg area would be roads. Our Interstate highway traffic congestion rosette extends as far out as Atlanta's, and we're hardly a world city.
It would be nice to have rail transit for commuters into our small urban nexus.

I've never seen interchanges anywhere else like they have here in the Twin Cities. They are dreadful and scary as Fhuc if you are just learning to drive! The one that, while not the worse in terms of danger, bothers me the most is I-35W and I-494. Two MAJOR interstate grinding to a halt to let mororists go from 65 to 20 mph to let other drivers on and off the other freeways. Unreal! Overhaul that ramp and add some capacity to I-494 and all of the sudden that area looks modern!
The ramp on exit 28 from I-91 for northbound access to Route 15 is always backed up about 1/2 - 1 miles, even on a Sunday afternoon. It's a bottleneck, because it's one of the only few ways to get from I-91 North to I-84 East. My suggestion would be to widen the ramp to two lanes.
I think Houston needs more rail, and a nice bridge to nowhere
