I think it's people following too closely, not everyone can be a NASCAR driver, just had a truck follow me on the freeway at 65 MPH, with not even a 2 second stopping distance between me and him, WTH?

not paying attention. saw it happen just the other day, dude wasnt paying attention and ran a red and guy at the other intersection smashed right into him

As a ranking...
1) Tailgating, too close to the car in front of your car

Between driving too fast and too slow, i think driving too slow is worse, since the chaos that comes when someone is driving too slow and everyone has to either slow down and pass them, is at times, very dangerous, especially on a freeway....

I would think recklessness and intoxication (alcohol, marijuana, other) would still be pretty high up there.

The umbrella classification of people being dumb accounts for, in my amateur opinion, 99% of accidents. Either driving aggressively because they feel the road belongs to them and them only, people driving recklessly because they're running late or in a hurry or whatever, or people driving in conditions beyond their capabilities (snow, ice, wind, etc.) my personal favorite, people too drunk to walk thinking they have the right to endanger every other person on the road because they're too lazy or cheap to get a cab. Very rare in this country is an accident that occurs entirely through misfortune and not irresponsibility.

I vote distractions as the number one cause of accidents......including overly loud music with "heavy" bass.

cell phones, i know its illegal in some areas to drive and text or talk, but i see teens mostly holding the phone low on their lap not even looking at the road for seconds at a time!
