Both true. That said, if I'm on the road and there's a Culver's, I'm having a burger and custard. If I'm in CA, I have no problem with an In-N-Out burger. There are a couple exceptions, but overall I'll take a one-of-a-kind local spot every time.

Considering that there is usually always a better alternative where I live and just about any city I travel to, yes it's all garbage. I'd rather dine at a locally run high caliber steakhouse (ex: Vernon's Hidden Valley in ABQ, Metropolitan Grill in Seattle) than Ruth's Chris or Mortons. Applebees and Chili's are mediocre at best, so many other pubs and restaurants to choose from where I could find a better and more memorable meal for the same price. Why go to PF Chang's when I could get a more authentic Szechuan meal at a better Chinese restaurant? And don't even get me started on Olive Garden and Papa John's, I'd trust that even small cities in Wyoming likely have better options for pizza and Italian and I would make an effort to look for them if I were there.

Hipsterism aside, most people eat at fast food restaurants. A lot of people just lie about it because this country's culture believes in shaming people into desired behavior.
Nonetheless, in the South, you have more it seems. Zaxby's, Chick Fil-A, Raising Cane's, Hardee's, Fatz, Dairy Queen, Firehouse Subs
In the North you have Tim Horton's and Boston Market.
In the west, you have In and Out Burger, Tastee Freez, Whattaburger.
I'm not saying that you can't have a few of each in other areas, just where the concentration lies.
