A clean downtown will be a boring downtown. Think Winnipeg, Tulsa, Charlotte, or something.
Every world class downtown I've visited (NYC, Paris, London, Rome, Tokyo, Hong Kong) was a little bit messy. Ok, maybe SF was a lot messy, but still. You need people and activity and a bit of urban tumult to have an impressive downtown.
It's not even that cleaning crews in San Francisco do a bad job, it's just that you have: Homeless people who use downtown as an encampment, jerks from Mid-Market who trash the place, and Chinatown inhabitants who do the best they can but are moving around too many things. The area near the trolley car turnaround is clean, but veer off the super-tourist area, and it's beauty covered in filth.
