This is weird because I was actually thinking about this today. I actually associate it with western PA and Amish country, sort of that eastern Ohio and western Pennsylvania rural area.

Obviously Pittsburgh and Phila are awesome cities that I would love to go to someday - and could probably live in. But for some reason I think of those cities apart from the "state" of Pennsylvania - which I associate with a more somewhat sophisticated proper-speaking rural culture.

Also I'm not sure why so many people turned this thread into a "What are the first two words that come to mind?" I don't think that was your point, OP, right?!
