I've been to every city on this list. The cleanest from my observations of all cities I've been to are (in no particular order) Boston, Charlotte, and Denver.
You must not have been in LoDo. Vomit splats everywhere from the drunks, who include both bums and suburbanites on weekend benders,
Why? Because the grid street layout allowed the creation of the nation's largest alley system (1900+ miles within Chicago). Back alleys are essential for the storing, collecting, and hiding of garbage (and other life unpleasantries), which helps keep the storefronts, pedestrian walkways, and tourist centers relatively pristine as compared to other big cities.
Don't know if this qualifies as major, but I was in Louisville's downtown for the first time Monday and there was an amazing amount of pedestrian traffic and street activity for such a small metro.
