NY's list does capture the two cities that cross county lines, however it also includes several villages (a lower-tier municipal government in NYS, an overlay on one or more towns), and a CDP (that's just cheating).

Brookline, MA is solely in Norfolk county but it is an exclave that doesn't touch the rest of the county at all. Off topic a bit but I thought it was kind of a cool bit of info.

maybe someone can answer the legal question on this one because I don't really know. Obviously San Francisco is a city and a county. SFO is located on the peninsula in San Mateo County and is not connected to the city. It's address is "San Francisco" and it is owned by the city and county of SF......but does that make it actually a part of San Francisco?
